CF Sports Awards Nomination Process
The CF Sports Awards were created to recognize the sports achievements of its serving and retired members, and the DND/PSP civilians employees that contribute to the CF Sports Program. This website has been created to explain the nomination process for the CF Sports Awards that are presented at the Annual CF Sports Awards Ceremony in the fall of each year (Hall of Fame, Honour Roll, and CF Sports Achievement Awards at the Environmental and CF Levels). Details about these awards, the categories covered by these awards, and the criteria for each award, can be found in the reference CFAO 50-17. This is an excellent resource to use when nominating a worthy individual as it provides background on all of the categories and explains the criteria for each.

Nominations:
The nomination forms that can be downloaded below have been created based on the details outlined in CFAO 50-17. This form has been posted here to act as a guide for those that would like to have one. Please be advised that a nomination submission does not have to be written up using this form, provided all of required information is received. If you do use the form, you will ensure the selection committee will be provided all of the required information for the nominee.
A nomination is not complete without the following photos of the nominee:

Note: ALL .jpg photos must be 300 dpi (no smaller as they may be required for use in the commemorative booklet and newspapers).
The PSP staffs on many bases are experienced in this nomination process. They will be happy to help out sponsors that need to arrange award nominee "photo shoots" with Base Photo Units. Please, wherever possible, ensure the photographer's name is provided so that credit is given, where credit is due.
Submission of Nominations
Achievement Award Winners - Nominations for Athlete, Coach, Official, or Team of the year must be sent to the Environmental Sports Representative of the nominee's environment in a timely manner. A selection board at the Environmental Staff Level will sit and submit the winner's names to the CF Sports Office no later than 30 April 2010, as per message sent January 2010. Please contact the appropriate Environmental Sports Representative for their submission deadlines. Please ensure that the nomination is complete. See the example nomination forms below for assistance.
Hall of Fame Nominations - Nominations for Induction into the CF Sports Hall of Fame must be submitted directly to the National Events Coordinator in the CF Sports Office no later than 30 April 2010, as per message sent January 2010. Please ensure that the nomination is complete. See the example nomination forms below for assistance. Any nominations with missing photos or information received after the 30 April 2010, as per message sent January 2010 will not be taken into consideration for that year.
Honour Roll Nominations - Nominations for Induction into the CF Sports Honour Roll must be submitted directly to the National Events Coordinator in the CF Sports Office no later than 30 April 2010, as per message sent January 2010. Any nominations with missing photos or information received after the 30 April 2010, as per email message January 2010 will not be taken into consideration for that year.

Form Description:
Section 1
Please ensure that all parts of this are included in the nomination process. The CF Sports Office must be able to get in contact with each nominee to ensure that all text for awards and write-ups are accurate.
Section 2
Please ensure that career dates include Reserve and Regular Force "time in" (if applicable). All awards require that the nominee be a member of the CF during their accomplishments. Please ensure that time on UN/NATO/Oversees tours are included.
Section 3
This section must be as complete and accurate as possible. It has been divided into levels of competition to ease in the selection process. These are the categories that are considered when deciding whether an individual qualifies for an award or induction into the Hall of Fame or Honour Roll. Please include whether they were a coach, official, athlete, or administrator for the entries in this section (the first column is provided for this distinction). Qualifications for Coaches and Officials are also important details to know. Consequently, a place has been provided for these qualifications.
Section 4
This narrative is not meant to repeat everything in section 3. It is meant to paint a picture of this athlete/coach/official/team for the selection committee. It should provide a bit of a background into why the nominee deserves to be recognized for their respective award. Many nominations include their commitment to their sport within the CF Sports Program, and their leadership role on their teams (captain, coach/player, etc.) where applicable. It is suggested that the narrative be no less than, but not limited to, 200 words in length.
Section 5
To ensure that the nomination is accepted this section must be completed in full so that if there are any questions the CF Sports Office or Selection Committee can contact the sponsor for clarification.
